HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Our duties
Seva Senior Services is required by law to protect the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. We must notify you if a breach affects the privacy or security of your information.
How we may use and disclose your information
- For treatment and care coordination. We share information with nurses, caregivers, coordinators, physicians, and other providers involved in your care.
- For payment. We use your information to determine Medicaid eligibility, obtain authorizations, submit claims, and receive payment for services.
- For health care operations. Quality review, care management, staff training, licensing, accreditation, and business planning.
- To family and others involved in your care. We may share relevant information with a family caregiver, personal representative, or a person you name, unless you object.
- As required by law. Including reporting suspected abuse, neglect, or exploitation of an older adult, public health activities, health oversight and Medicaid audits, judicial and administrative proceedings, law enforcement requests permitted by law, and to avert a serious threat to health or safety.
Uses that always require your written authorization
We will never sell your PHI, and we will not use it for marketing or fundraising, or share psychotherapy notes, without your written authorization. You may revoke an authorization at any time in writing, except to the extent we have already acted on it. Photographs or testimonials of clients are used only with signed permission.
Your rights
- Inspect and copy your records, usually within 30 days of a written request. A reasonable, cost-based fee may apply.
- Ask us to correct information you believe is wrong or incomplete.
- Request confidential communications, such as calls to a specific number or mail to a different address.
- Request restrictions on certain uses and disclosures. We must agree to a restriction on disclosure to a health plan for a service you paid for entirely out of pocket.
- Receive an accounting of certain disclosures we have made in the previous six years.
- Get a paper copy of this notice at any time, even if you received it electronically.
- Choose someone to act for you. A guardian or holder of a health care power of attorney may exercise these rights on your behalf.
- File a complaint without fear of retaliation.

You may also complain to the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, DC 20201, or by phone at 1-877-696-6775. We will not retaliate against you for filing a complaint.
Changes to this notice
We may change this notice and apply the revised terms to information we already hold. The current notice will always be posted on this page, with its effective date, and available at our office.
