Seva Senior Services (“Seva,” “we,” “us”) is a Georgia Medicaid provider offering paid family caregiving support, a senior day center, and private-pay home care. Information we collect
We collect information you give us directly through this website, by phone, by email, or during intake, including:
- Contact details such as your name, phone number, email address, and county of residence.
- Information about your caregiving situation and your loved one’s care needs, which you choose to share so we can assess eligibility.
- Information needed to determine Medicaid eligibility and to complete enrollment, which may include Medicaid identifiers and clinical or functional information.
- Basic technical information collected automatically, such as browser type, device type, referring page, and pages viewed.
How we use your information
- To respond to your inquiry and contact you about services you asked about.
- To assess eligibility and to prepare, submit, and support Medicaid waiver applications and renewals on your behalf.
- To coordinate and deliver care, including nurse visits, care coordination, and day center services.
- To meet legal, regulatory, licensing, billing, and audit obligations.
- To improve our website and the clarity of our intake process.
We do not sell your personal information, and we do not use protected health information for advertising.
When we share information
We share information only as needed to provide services and as permitted by law: with the Georgia Department of Community Health and its designated agencies and care management organizations for eligibility and enrollment; with clinicians and caregivers involved in your loved one’s care; with service providers who support our operations under written confidentiality obligations; and when required by law, subpoena, or a government audit. Where the information is protected health information, our HIPAA Notice of Privacy Practices governs how we use and disclose it.
Text messages and calls
When you submit a form or text us, you agree that we may contact you by phone, text, and email about your inquiry. Message and data rates may apply. You can opt out of text messages at any time by replying STOP, or ask us to stop contacting you by calling (678) 671-0500.
