By using sevaseniorservices.com or submitting a form on it, you agree to these terms. If you do not agree, please do not use the site. These terms cover use of the website only. Services we deliver to you are governed by the separate service agreement and care plan you sign with us.

No guarantee of eligibility or payment

Eligibility for Georgia Medicaid waiver programs, including structured family caregiving, is determined solely by the Georgia Department of Community Health and its designated agencies. Seva prepares and supports applications and enrollment, but we cannot guarantee approval, a timeline, a specific rate of pay, or a specific benefit amount. Program rules, rates, and availability may change without notice.

Forms, calls, and texts

Submitting a form is a request for contact, not an enrollment. By submitting, you consent to be contacted by phone, text, and email about your inquiry. Message and data rates may apply. Reply STOP to opt out of texts. Do not submit sensitive medical details through a public form. Call us instead at (678) 671-0500.

Your responsibilities

• Provide accurate, complete information during inquiry and intake.
• Do not submit information about another person unless you are authorized to do so.
• Do not use the site to send unlawful, misleading, or harmful content, or attempt to disrupt or gain unauthorized access to it.

The Seva name, logo, site design, text, and images are owned by Seva Senior Services or licensed to us, and may not be copied or reused without written permission.

We link to other sites, including Seva Home Care Illinois and state resources, for convenience. We are not responsible for their content or practices.

Disclaimer and limitation of liability

The site is provided on an “as is” and “as available” basis without warranties of any kind. To the fullest extent permitted by law, Seva is not liable for indirect, incidental, or consequential damages arising from your use of the site. Nothing here limits any right you have under your signed service agreement or applicable law.
